APPLICATION FOR FAIR QUEEN PAGEANT

Be sure to fill out this application completely. Please include a head and shoulder view photograph. It is imperative that this photo be as clear as possible. Proofs or computer-generated pictures may not be used. Your photograph will be returned to you after the contest.

Please complete and return this application by Friday, June 11, 2010, along with your picture and $30 entry fee to: MORGAN COUNTY FAIR, 1749 Hospital Drive, P.O. Box 1534, Martinsville, IN 46151. The entry fee and picture must be received with the application. The fair office is open on Mondays and Fridays, from 9:00 a.m. till 4:00 p.m. (possibly other days). If you have any questions, please phone 765-342-9905. Attendance will be taken at each practice and the judges will be informed of which practices and how many practices you attended. The final practice is mandatory.
